NEW PROCEDURE FOR ATHLETIC CLEARANCE EFFECTIVE SUMMER 2009


According to CIF regulations, all athletes must obtain an annual physical and medical release from a physician prior to participating in athletics, including summer participation (effective summer 2009).

Saturday, May 30th at the Agoura High School Gym
Sports Physicals and Clearance Day
8:00 am - 12:00 noon


The Cost is $25

Registration for Grade 9: 8:00 am
Registration for Grade 10: 9:00 am
Registration for Grade 11: 10:00 am
Registration for Grade 12: 11:00 am

Saturday, May 30, 2009 is the annual clearance day for all AHS new and returning athletes. AHS will offer athletes the opportunity to obtain their medical clearance on this day. The cost for a physical is $25 and must be paid when the physical is given. Checks are to be made to Agoura High School. All fees are donated to the AHS Athletic Program. Students will also have the option to obtain a physical fom their own physician; however, the completed athletic packet must be turned in on this day.

SUMMER/OFF-SEASON CLEARANCE

 

You must submit the following forms on Saturday, May 30th at the Sports Physicals and Clearance Day
(All forms must be turned in at one time)
  1. ACTIVITY CERTIFICATE: (page 1.)
  2. MEDICAL RELEASE: (page 2.)
  3. CO-CURRICULAR CODE AND CONTRACT: (page 3.)
  4. ATHLETES CODE OF ETHICS: (Page 4.)
  5. VOLUNTARY ACTIVITIES PARTICIPATION FORM: (page 5.)
  6. DRUG TESTING POLICY/CONSENT: (Page 6.)
  7. EMERGENCY MEDICAL CARD: Summer/Off-Season
  8. EMERGENCY MEDICAL CARD: School Year Season of Sport - A seperate emergency card must be submitted for each sport. Cards cannot be transferred from one sport to another.

A make-up day is scheduled to submit completed athletic packets only on June 6th from 9:00 - 10: 00 am in the Activity and Athletic Center.

Summer athletic participation fees are set by the district. All summer fees should be payable to Agoura High School and submitted to the coach.

SCHOOL YEAR SEASON CLEARANCE

A completed athletic packet is required to tryout for a sport during the school year. If summer or off-season clearance was previously obtained, only a new emergency card is required for tryouts during the school year. If clearance was not previously obtained, a completed athletic packet including a medical release from a physician, (see above) must be submitted to the Activity and Athletic Center prior to trying out for a sport.

AFTER TRYOUTS AND TEAM SELECTION

  • Become a member of the Associated Student Body for the current year: Most students join the ASB, as this provides free admission to athletic events(except CIF playoffs), and discounts for numerous school activities during the year.) If you are a member, bring your ID Card with you for proof. If you are not a member, you can join for $55.00. Please make checks payable to Agoura High School.
  • Make an athletic donation of $150.00 per sport . This donation covers transportation costs, the athletic trainer, and facility maintenance. If the donation presents a financial problem, special arrangements can be made with the Assistant Principal of Athletics at (818)889-1262 Ext. 237 (please note that there is a $400.00 maximum donation per student or per family annually. Once that amount has been paid, please inform the Activity and Athletic Center. Please make checks payable to Agoura High School.
