All students who wish to participate in athletics are required to submit a completed Athletic Packet, including an annual physical and medical release from a physician, to the Activity and Athletic Center. CIF requires clearance for tryouts, off-season, summer and school year participation.

The Athletic Packet contains the following forms for your information only:

A. Code of Conduct Contract
B. Code of Conduct (Part 2)
C. Drug Testing Policy
D. Drug Testing Policy (Part 2)
E. NCAA Clearinghouse Information

The following forms are to be completed, printed and submitted to the AAC for clearance:

1. Activity/Athletic Certificate
2. Health History
3. Code of Conduct – Signature Form
4. Athlete’s Code of Ethics – Signature Form
5. Voluntary Activities Participation Form
6. Consent to Test Form – Drug Policy
7. Emergency Card – School Year Season of Sport
8. Emergency Card - Summer/Off-Season/Tryout
(Both emergency cards must be submitted to obtain clearance.  New emergency cards must be submitted for each sport. Cards cannot be transferred from one sport to another.)

AFTER TEAM SELECTION

• Become a member of the Associated Student Body for the current year: Most students join the ASB, as this provides free admission to athletic events(except CIF playoffs), and discounts for numerous school activities during the year.) If you are a member, bring your ID Card with you for proof. If you are not a member, you can join for $60.00. Please make checks payable to Agoura High School.
• Make an athletic donation of $150.00 per sport . This donation covers transportation costs, the athletic trainer, and facility maintenance. If the donation presents a financial problem, special arrangements can be made with the Assistant Principal of Athletics at (818)889-1262 Ext. 237 (please note that there is a $400.00 maximum donation per student or per family annually. Once that amount has been paid, please inform the Activity and Athletic Center. Please make checks payable to Agoura High School.
