NEW PROCEDURE FOR ATHLETIC CLEARANCES
According to CIF regulations, all athletes must obtain an annual physical and medical release from a physician prior to participating in athletics, including tryouts and off-season participation.
TRYOUTS/OFF-SEASON CLEARANCE
You must submit the following forms to the AAC prior to trying out or participating in any sport at Agoura High School.
- ACTIVITY CERTIFICATE: (page 1.)
- MEDICAL RELEASE: (page 2.)
- CO-CURRICULAR CODE AND CONTRACT: (page 3.)
- ATHLETES CODE OF ETHICS: (Page 4.)
- VOLUNTARY ACTIVITIES PARTICIPATION FORM: (page 5.)
- DRUG TESTING POLICY/CONSENT: (Page 6.)
- EMERGENCY MEDICAL CARD: Tryout/Off-Season/Summer
- EMERGENCY MEDICAL CARD: School Year Season of Sport - A seperate emergency card must be submitted for each sport. Cards cannot be transferred from one sport to another.
AFTER TEAM SELECTION
- Become a member of the Associated Student Body for the current year: Most students join the ASB, as this provides free admission to athletic events(except CIF playoffs), and discounts for numerous school activities during the year.) If you are a member, bring your ID Card with you for proof. If you are not a member, you can join for $55.00. Please make checks payable to Agoura High School.
- Make an athletic donation of $150.00 per sport . This donation covers transportation costs, the athletic trainer, and facility maintenance. If the donation presents a financial problem, special arrangements can be made with the Assistant Principal of Athletics at (818)889-1262 Ext. 237 (please note that there is a $400.00 maximum donation per student or per family annually. Once that amount has been paid, please inform the Activity and Athletic Center. Please make checks payable to Agoura High School.
