Registration Procedures for New Enrollees

* Apointment must be scheduled to enroll - No walk-ins *

1. To make an apointment please call Mrs. Wortham at (818) 889-1262 X 246 We will start taking phone calls to make apointments as of May 1, 2008. The apointment dates will be for May 27th-May 30th.

2. Students must have a parent or gardian with them at the appointment to register. Gardians must have guardianship papers.

3. Prior to the apointment parents must apply for a Inter/Intra district permit with the district office if they do not live within the attendance area.

What you need to bring with you to your appointment

at AHS in the counseling/registrar's office in May

► A copy of the students transcript

► A copy of the students IEP if applicable

► Proof of residency using a utility bill (no telephone bills).

► Students current immunization record

► Students birth certificate or passport
